Please note on DVH for $ 2,000 clients would receive $ 1,300.00 1st year and $ 1,500 2nd year & thereafter for life of policy

IF  DVH for $ 2,500 clients would receive  $ 1,625.00 1st year and $ 1,875 2nd year & thereafter for life of policy

Some Broker cross selling for both states ( IN. ) would have greater % amounts coming to client or we pay provider–either way for insured.
